Instance 1- Employees were exposed to fall hazards when an extension ladder utilized to access a ladder jack  scaffolding did not extend 3 feet above the upper landing surface.  Instance 2- Employees were exposed to fall hazards when an extension ladder utilized to access the roof did not extend 3 feet above the upper landing surface.

29 CFR  1926.451(e)(1):scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross Braces shall not be used as a means of access.  On April 6, 2023 employees were exposed to hazards associated with falls when the employer did not provide a safe point of access to a platform that was more than 2 feet above the point of access. Employee were climbing over and around bracket and platform to access the work area.

The Star Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1489558, citation number 1, item number 1 and was affirmed as a final order on 10/13/2023, with respect to a workplace located at the Epsom Traffic Circle in Epsom NH. 03234.
