29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part.   a) At 745 Arbor Estates Way, Plant City, FL:  On or about April 11, 2023, the employer exposed employees to fall  hazards, in that, employees were performing roofing work during which time the employer failed to initiate and maintain safety and health program, as necessary to comply with fall protection and ladder safety.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     a) At 745 Arbor Estates Way, Plant City, FL:  On or about April 11, 2023 the employer exposed employees to fall hazards, in that, access portable ladder was not extended 3 feet above the upper landing surface and not secured.   1) One portable aluminum extension ladder next to the loading machine, south side of building.  2) A second access portable aluminum extension ladder at the middle of front side, south side of building.  3) A third portable aluminum extension ladder installed in the east side of building.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.    a) At 745 Arbor Estates Way, Plant City, FL:  On or about April 11, 2023, the employer exposed employees to fall hazards, in that, ladder training was not provided to employee using ladders to access/descend from the roof.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems.  a) At 745 Arbor Estates Way, Plant City, FL:  On or about April 11, 2023, the employer exposed employees to fall hazards, in that, fall protection was not utilized by employees while performing roofing work.  Roofers Hub LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1476357, citation number 1, item number 1 and was affirmed as a final order on 08/20/2020, with respect to a workplace located at 222 Stillwater Avenue, Billings, MT, 59105.
