On or about April 10, 2023, a competent person did not conduct inspections of an excavation to ensure conditions did not exist that could result in possible cave-ins, exposing employees to struck-by hazards.

On or about April 10, 2023,  employees were not provided protective systems when working in an excavation that was more than 5 feet in depth. The employees were exposed to soil collapse and struck-by hazards.
