29 CFR 1915.74(a)(1): A gangway of not less than 20 inches walking surface of adequate strength, maintained in safe repair and safely secured was not used. If a gangway is not practical, a substantial straight ladder, extending at least 36 inches above the upper landing surface and adequately secured against shifting or slipping was not provided. When conditions are such that neither of gangway nor a straight ladder can be used, a Jacob's ladder meeting the requirements of (d)(1) and (2) were not used. M/V Super Express: On and/or before April 11, 2023, employee(s) conducted basic engine repower, engine alignments, final torques and bilge cleaning. A gangway, substantial straight ladder, or Jacob's ladder was not used for safe entry or exit of the vessel. The distance from the vessel to the white Todd Triple Boarding Steps measured to be approximately 16 inches from the vessel and 19 inches in height from the top step to the entrance of the entrance of the vessel. This condition potentially exposed employee(s) to slip, trip, fall, and drowning hazard(s).
