On or about April 4, 2023, the employer did not ensure the energy control procedure established for the Gluer Machine #916 clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance. An employee  was exposed to nip point hazards and was injured when re-aligning the belt on the machine and using their hand while the machine was energized.
