29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces. Jobsite - On or about April 11, 2023, a Werner 24-foot fiberglass extension ladder was used by an employee for access to the roof, where the slip-resistant feet were placed on a piece of cardboard on a composite deck surface. The employee suffered a serious leg injury when the ladder slipped and kicked out, causing the employee to fall approximately 16 feet.

Jobsite - On or about April 11, 2023, the employer did not use accident prevention tags as a temporary means of warning employees of a hazard with the Werner 24-foot extension ladder, which was set up on a composite deck surface with cardboard under the slip resistant feet and had not been secured to prevent displacement.
