29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) West side of residential structure; A Keller brand model KMT-22 multi-position aluminum ladder placed on the rear deck of a single-family home  and used as an extension ladder by employees to access and egress the roof, was not adjusted to allow for the side rails to extend at least 3 feet above the landing surface and was not equipped with a grasping device(s).
