29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: Location: 44 Bedford St., Middleboro, MA 02346 On April 12th, 2023, employees were exposed to falls from heights up to 20 feet to the ground while performing wood framing activities where fall protection was not utilized.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. Location: 44 Bedford St., Middleboro, MA 02346 On April 12th, 2023, employees were exposed to falls from heights up to 16 feet to the ground while accessing the roof where the employees were climbing from an extension ladder, onto the inside of an exterior wall, and up through the trusses to access the roof and complete wood framing activities.
