29 CFR 1910.36(g)(2):An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access. (a) On or about April 12th, 2023, the exit route from the part storage room was limited in width to 21 inches due to the storage of materials within the exit route.

29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a)On or about April 12, 2023 the WC-15 Wheel Cleaner had an interlock defeated on the door allowing the operator to access the point of operation. This exposed the operator to amputation hazards.
