29 CFR  1926.451(f)(3): Scaffolds and scaffold components were not inspected visible defects by a competent person before each work shift or after any occurrence that could affect a scaffold's structural integrity:   a) Job site, 7507 McKnight Road, Pittsburgh, PA: On or about April 12, 2023, during construction activities at the site, the employer did not ensure that the scaffolding in use was properly inspected by a competent person for visible defects, all of which exposed employees to fall hazards of up to approximately 25 feet. Deficiencies noted during the inspection included, but were not limited to, failing to provide fall protection at the end of a scaffold frame, relying only on a single outrigger bracket and no midrail for fall protection along the end of a scaffold outrigger, using cross bracing alone as fall protection where it's center point from the work platform was determined to be 46 inches and no midrail was provided, permitting guardrails that were not of adequate height to be used as fall protection between 2 scaffold sections and allowing guardrails systems to be far away from the work platform horizontally which created a fall hazard.

29 CFR  1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:   Instances including, but not limited to, the following:  a) Job site, 7507 McKnight Road, Pittsburgh, PA: On or about April 12, 2023, during construction activities along the front of the building, the employer did not ensure that employees had guardrails installed along the end of a scaffold, exposing an employee to a fall hazard of up to approximately 19 feet.  b) Job site, 7507 McKnight Road, Pittsburgh, PA: On or about April 12, 2023, during construction activities along the side of the building, the employer did not ensure that employees had appropriate fall protection in use. The employer permitted the use of just a single outrigger bracket to be used for fall protection along the end of the scaffold, exposing an employee to a fall hazard of approximately 13 feet.

29 CFR  1926.451(g)(4)(xv): The employer did not ensure that a midrail was in use where cross bracing was permitted to serve as fall protection and it's center point was 46 inches above the work platform:    Instances including, but not limited to, the following:  a) Job site, 7507 McKnight Road, Pittsburgh, PA: On or about April 12, 2023, during construction activities along the site of the building, the employer did not ensure that appropriate fall protection was in use where a fall hazard of approximately 25 feet existed in that there was no midrail in use where the employer permitted a cross brace to serve as fall protection and it's center point was determined to be 46 inches above the work platform.  b) Job site, 7507 McKnight Road, Pittsburgh, PA: On or about April 12, 2023, during construction activities along the side of the building, the employer did not ensure that appropriate fall protection was in use where a fall hazard of approximately 13 feet existed in that there was no midrail in use where the employer permitted a cross brace to serve as fall protection and it's center point was determined to be 46 inches above the work platform.
