29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation  (a) On or about April 12, 2023, employees were operating  pneumatic nail guns without eye protection while installing roof shingles,  exposing employees to a struck by hazard and eye injury.

29 CFR 1926.300(b)(2):  Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment shall be guarded if such parts are exposed to contact by employees or otherwise create a hazard. Guarding shall meet the requirements as set forth in American National Standards Institute, B15.1-1953 (R1958), Safety Code for Mechanical Power-Transmission Apparatus.  (a) On or about April 12, 2023, employee(s) used an extension ladder hoist to convey materials up to the roof. The hoist did not have the power transmission pulley and belt guarded, exposing employees to a caught-in hazard and amputation injury.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about April 12, 2023, employees conducting roofing work on the south side of an existing roof with a 8:12 pitch, were not wearing fall protection. This exposed employees to a fall hazard of approximately 9 feet from roof to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about April 12, 2023, employees installing shingles on a new residential home, used extension ladders to access the roof that were not erected three (3) feet above the upper landing. This exposed employees to a fall hazard of approximately 9 feet to the ground below.
