On or about April 12, 2023, at the facility, the employer had not posted danger signs of the existence and location of the danger posed by the permit required space. The locations where the danger signs were not posted were the following: 1. Access vault between silos #16 and #17 2. Access vault between silos #18 and #19 3. Access Doors into Grain Silos #16, #17, #18 and #19
