29 CFR  1910.1200(e)(1):Employer did not  implement  and maintain a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   a) At the workplace,   where employees are exposed to hazardous chemicals, the employer did not update the written hazard communication program to document the update from material safety data sheets (MSDS) to safety data sheets (SDS). 29 CFR  1910.1200(h)(1):Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) At the workplace,   Employees exposed to hazardous chemicals such as: Ecolab Performance containing hydrogen peroxide and acetic acid, E-max Alkali containing sodium hydroxide, Injection Sour containing formic acid, did not receive hazard communication training.  On or about April 12, 2023.

29 CFR  1904.29(b)(1):The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300:  At the workplace, Section F of the OSHA 300 log for 2022 was not completed as required. The employer  listed the body part injured  but not the injury or illness.  On or about May 12, 2023.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
