29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 29704 Meadow Spur Lane, Evergreen, CO 80439: On April 10, 2023, H2 Plumbing & Heating, Inc. had an employee fall while using an unauthorized ladder which resulted in the employee breaking their leg. The employee was then hospitalized, and H2 Plumbing & Heating, Inc reported the employee hospitalization to OSHA on April 12, 2023.
