Located at 1250 Clover Street, Sunbury, Ohio: On or about April 13, 2023, the employer did not maintain an effective safety and health program to ensure that hazards were identified and corrected onsite while employees performed residential roofing activities, thereby exposing employees to struck by hazards and fall hazards from up to approximately twenty-three feet.

29 CFR 1926.502(d):"Personal fall arrest systems." Personal fall arrest systems and their use shall comply with the provisions set forth below. Effective January 1, 1998, body belts are not acceptable as part of a personal fall arrest system. Note: The use of a body belt in a positioning device system is acceptable and is regulated under paragraph (e) of this section. a. Located at 1250 Clover Street, Sunbury, Ohio: On or about April 13, 2023, the employer did not ensure that employees were provided with a compliant personal fall arrest system while performing residential roofing work. Employees were using various unapproved climbing harnesses as personal fall arrest equipment that were attached to rope lifelines wrapped around the nail plate at the roof truss peak and tied to itself, thereby exposing employees to falls from approximately twenty-three feet.
