29 CFR  1910.147(c)(7)(i)(A):  Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  Production Floor:  On, or about, April 5, 2023, employees who performed servicing and maintenance tasks on Machine #61 such as removing guards, cycling the machine to identify the source of an abnormal noise and purging the machine with guards removed had not received training regarding lockout/tagout policies and procedures.

29 CFR  1910.1030(d)(4)(ii)(A):  Contaminated work surfaces were not decontaminated immediately or as soon as feasible when surfaces were overtly contaminated or after any spill of blood or other potentially infectious materials:  Company:  On, or about, April 5, 2023, the company did not provide an appropriate disinfectant to be used to clean blood or other potentially infectious materials following an injury to an outside contractor.  The disinfectant cleaner that was provided was diluted and was not left on contaminated surfaces for the contact time required and the isopropyl alcohol that was used is classified as an antiseptic and is not registered as an EPA disinfectant to clean blood borne pathogens.
