29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  On or about April 13, 2023, employees were exposed to a struck-by hazard while using a pneumatic nail gun without eye protection.

29 CFR  1926.501(b)(1): Employee on a walking/working surface with an unprotected side or edge which is 6 feet or more above a lower level were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about April 13, 2023, employees without a fall protection system were exposed to a fall hazard exceeding 6 feet while installing trusses.
