29 CFR 1910.268(o)(1)(i): When covers of manholes or vaults were removed, the opening was not guarded by a railing, temporary cover, or other suitable temporary barrier which is appropriate to prevent an accidental fall through the opening and to protect employees working in the manhole from foreign objects entering the manhole: Jobsite located at the junction of Elm Street and South Center Street, Windsor Locks, Connecticut: On, or about, April 10, 2023, an employee accidently stepped into an open, unguarded manhole while gathering fiberoptic cable to be dropped back into the manhole vault at the conclusion of the job. At the time of the incident, the manhole was not provided with a gate or other suitable protective device.
