29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On April 13, 2023, employees of CKA Construction LLC were installing roofing shingles on a residential building located at 500 Hawthorne Ridge Court in Foristell, Missouri. Metabo HPT, NV45AB2, pneumatic nail guns were being used without wearing eye protection. The employees were exposed to struck-by hazards.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: On April 13, 2023, employees of CKA Construction LLC were installing roofing shingles on a residential building located at 500 Hawthorne Ridge Court in Foristell, Missouri. The Owner and employees were working on the roof of the building approximately 11 feet above the lower level. The employees did not utilize guardrail systems, a safety net system, or a personal fall arrest system.
