29 CFR 1926.451(b)(4):Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm) a) The Grove Assisted Living, 2425 McMullen Booth Rd., Clearwater, FL - employees in the process of tying in rebar while working on the top work platform of a tubular welded frame scaffolding system, 3 levels high measuring 6'4" tall by 5 foot wide by 7 feet in length sections, were exposed to a 19'2" fall hazard, in that, the 6 scaffold planks did not extend over the centerline at least 6 inches. Scaffold planks measured between 7'9" to 8'5" in length.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) The Grove Assisted Living, 2425 McMullen Booth Rd., Clearwater, FL - employees in the process of tying in rebar while working on the top workplatform of a tubular welded frame scaffolding system, 3 levels high measuring 6'4" tall by 5 foot wide by 7 feet in length sections, were exposed to a 19'2" fall hazard, in that, the employer did not provide employees with hazard recognition training.
