29 CFR  1910.176(a):Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances was not allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked.  On or about  April 14, 2023, employer did not allow sufficient clearance space in the aisles for forklift operations, and the aisles and passageways were not appropriately marked for pedestrian or forklift transit.  These conditions exposed employees to struck-by hazards.
