29 CFR 1926.501(b) (13): The employer did not ensure that each employee engaged in residential roofing activities with unprotected sides and edges (6) feet or more above lower levels, was protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. a) On April 14, 2023, employees engaged in residential roofing were not protected from fall hazards while working at an elevation of approximately 18 feet above the ground. The 3 Star Shield, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1501302 citation number 1 item number 2 and was affirmed as a final order on December 30, 2020, with respect to a workplace located 694 East Oakton Avenue Desplaines, IL. 60016
