29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, shall be guarded to eliminate the hazard of impalement. Next to the road: Employees were working next to and above unguarded protruding reinforcing steel to remove seawater from the wastewater project trench. Employees were exposed to impalement hazards.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth: Between the ocean and the road in Aua: A trench approximately 20 feet in length, 6 feet wide and 4 feet deep did not have a ladder, acceptable sloped earth, ramp, or other safe means of egress. Employees were exposed to fall hazards. The Fletcher Construction Company Ltd was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1365541, citation number 1, item number 1 and was affirmed as a final order on 3/29/2019, with respect to a workplace located at Leloaloa Village, PAGO PAGO, AS .
