Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. On or about 13 April 2023, employees of Above Quality Roofing & Contractors LLC were exposed to fall hazards while working on the roof of a residential building located at 3596 Barter Street in Saint Charles, Missouri. The employees were installing roofing materials with improper fall protection.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about 13 April 2023, employees of Above Quality Roofing & Contractors LLC were exposed to fall hazards at 3596 Barter Street in Saint Charles, Missouri. The employees were installing roofing materials on the second-story of a residential building. The employee accessed the roof using a ladder that was not extended 3 feet beyond the upper landing surface.
