29 CFR 1926.1053(b)(1): Portable ladders are used for access to an upper landing surface and the ladder side rails do not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:   On or about 04/14/2023, the employer did not ensure portable ladders were constructed and placed so three (3) feet or more was extended above the upper landing.  Workers performing commercial construction were exposed to falling approximately eleven (11) feet while using a straight ladder to ascend/descend from the ground level to the roof.

29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about 04/14/2023, the employer did not protect employees who were engaged in commercial roofing operations 6 feet (1.8 m) or more above lower levels. The employees were exposed to the hazard of falling approximately eleven (11) feet and fall protection was not used.  Oliver Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard , 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1521949, citation number 1, item number 1 and was affirmed as a final order on 07/07/2021, with respect to a workplace located at 2 Parsons Creek Ct, O Fallon, MO 63366.
