29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   (a) On and before April 14, 2023, the employer did not develop, document, or utilize procedures at the workplace to control hazardous pneumatic, electric, and thermal energy during servicing and cleaning of Cartoner A, PMI Cartoning, Inc. horizontal cartoner, Model No. HSP-6, Serial No. 1144 . Employees performed servicing on Cartoner A, where the unexpected energization, startup or release of stored energy could occur and cause injury.   (b) On or about April 14, 2023, the employer did not develop, document, or utilize procedures at the workplace to control hazardous electrical energy during servicing, cleaning, and sanitation of the Peerless Cookie Dough Mixer on the Center line.  Employees performed cleaning and sanitation on the Cookie Dough Mixer on the Center Line, where the unexpected energization, startup or release of stored energy could occur and cause injury.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  (a) On or about April 14, 2023, the employer did not provide authorized user training to employees that would perform servicing and maintenance on Cartoner A, as a result, energy controls were not utilized to control hazardous energy sources during servicing, exposing workers to hazardous energy sources.  (b) Between April 14, 2023 and June 14, 2023, the employer did not provide authorized user training to employees that would perform cleaning and sanitation on the Center Line Cookie Dough Mixer in order to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application and usage of Lockout/Tagout to control hazardous energy sources.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or about June 14, 2023, the employer did not ensure that operators were protected from accessing moving parts of the caramel addition machine during the actuation of a pneumatic plunger, exposing workers to caught-in hazards.
