Spider Construction, 394 Malden Road, Coal Center, PA 15423: On or about April 17, 2023, while interviewing the employer and employee, the attachment point of the body harness was not located in the center of the employees back near shoulder level. The employees were inappropriately connecting the fall protection rope to the front of the harness.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer: a.) Spider Construction, 394 Malden Road, Coal Center, PA 15423: On or about April 17, 2023 a employee fell approximately 20 feet off a roof onto the below concrete driveway. The employer did not prepare a written certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.
