29 CFR  1917.50(c)(1): Each crane and derrick was not tested as a unit quadrennially and examined annually:   (a) on or about 3/31/2023 and at times prior thereto workers on the Pacific Seafood dock located at 316274 Lower Harbor Road, Brookings, Oregon were exposed to struck by hazards from a shore crane which could have failed because it had been taken out of service and was not subsequently examined and certified as safe for use by a third party as documented by an OSHA-71 form.

29 CFR  1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:   (a) on or about 3/31/2023 and at times prior thereto workers on the Pacific Seafood dock located at 316274 Lower Harbor Road, Brookings, Oregon were exposed to minor burn  hazards in that the fire extinguisher in the dock shed was not inspected annually.
