On a 6:12 pitched roof of a commercial worksite, located at 1998 S Fletcher Avenue, (Sliders Seaside Grill) Fernandina Beach, FL 32034: On or about April 18, 2023, the employer exposed its employees to fall hazards ranging from 6 feet to 30 feet, in that fall protection was not utilized by employees while repairing the roof.

On the roof of a commercial worksite, located at 1998 S Fletcher Avenue (Sliders Seaside Grill) Fernandina Beach, FL 32034: On or about April 18, 2023, the employer exposed its employee to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using screw gun and rivet gun to drill holes in the flashing.

On the roof of a commercial worksite located at 1998 S Fletcher Avenue, (Sliders Seaside Grill) Fernandina Beach, FL 32034: On or about April 18, 2023, the employer exposed an employee to fall hazards, in that the employee used the top step of a 6-foot A-frame step ladder to gain access and egress from the roof and was being exposed to fall hazards. b. On the roof of a commercial worksite located at 1998 S Fletcher Avenue, (Sliders Seaside Grill) Fernandina Beach, FL 32034; On or about April 18, 2023, the employer exposed an employee to fall hazards, in that the employee used the 6-foot A-frame step ladder in the closed position leaned against the roof.
