29 CFR 1926.501(b)(11):   Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a. On a second story 6:12 pitched roof, at a temporary worksite located at 6045 A1A South in Saint Augustine, Florida: On or about April 18, 2023, the employer exposed subcontracted employees to fall hazards ranging from 18 to 28 foot, in that in that fall protection, was not utilized, while performing roofing work.  Roof Pros Storm Division, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1534908, citation number 1, item number 1, and was affirmed as a final order on 10/20/2021, with respect to a workplace located at 979 Garrison Drive, Saint Augustine, FL 32080.

29 CFR  1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.   While accessing a second story 6:12 pitched roof, at a temporary worksite located at 6045 A1A South in Saint Augustine, Florida:  On or about April 18, 2023, the employer exposed subcontracted employees to fall hazards ranging from 6 to 18 foot, in that the side rails of a potable ladder were not extended at least 3 feet above the upper landing surface the roof, when employees climbed or descended the ladder.
