While accessing a second story 6:12 pitched roof, at a temporary worksite located at 6045 A1A South in Saint Augustine, Florida: On or about April 18, 2023, the employer exposed employees to fall hazards ranging from 6 to 18 foot, in that the side rails of a potable ladder were not extended at least 3 feet above the upper landing surface the roof, when employees climbed or descended the ladder.
