Location: 11 Felt Street, Salem, MA On or about 04/18/2023, employees were not protected from electric shock and were working less than five feet from live electrical home service lines.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours. Location: 11 Felt Street, Salem, MA On or about 04/20/2023, an emailed request for the previous 3 years of OSHA 300/300A logs, work contract between "Coastal Windows & Exteriors Inc" and "WMM Service Inc.", and applicable training records was made to the employer, the employer acknowledging receipt of the request but has since failed to provide the previously referenced documents. A reminder of this request was made during the closing conference call on 04/22/2023 but the employer has since failed to provide the requested information despite multiple emailed requests for information.
