OSHA opens inspections for many reasons — routine scheduling under a national or local emphasis program, an employee complaint or referral, or a follow-up after a reported injury. 1910.147(d)(1): Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy. On April 12, 2023, an employee was injured after performing lockout/tagout on the Embosser/Laminator unit operation on the GT2 Line. After the scheduled lockout/tagout procedure was completed on the Embosser/Laminator, employees removed a valve plate on the Embosser/Laminator Glue Assembly, an action which explicitly deviated from the Embosser/Laminator GT2 Line?s Lockout/Tagout Procedures. Not fully understanding the magnitude of the pneumatic energy to which he could be exposed when reenergizing the Embosser/Laminator Glue Actuator, the employee activated the equipment without removing his hand from the point of operation, an action which resulted in an injury.
