29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 19, 2023, the Accurshear, model 650012, serial #5294, had a barrier guard with a gap beneath the guard allowing access to the point of operation and material hold devices. Additionally, the back side of the shear was not guarded to prevent access to the point of operation and material discharge shoot, thereby exposing employees to an amputation hazard and struck by hazard.
