29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. On or about March 22, 2023, at an establishment located at 344 N Henry St, in Green Bay, Wisconsin, an employee of Gauthier and Sons' Construction, Inc. was exposed to fall hazards of greater than 4 feet and had not been trained to identify or minimize the hazards associated with falls.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about March 22, 2023, at an establishment located at 344 N Henry St, in Green Bay, Wisconsin, an employee of Gauthier and Sons' Construction, Inc. was hospitalized from an injury sustained from a fall and the hospitalization was not reported within 24 hours.
