29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) At 2312 Sparta Way Buford, Georgia, on or about April 19, 2023 where the groundman was observed exposed to struck-by and head injury hazards while handing sheets of plywood to the upper level, in that the use of hard hats was not enforced.

29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) At 2312 Sparta Way Buford, Georgia, on or about April 19, 2023 where the roofers were observed exposed to struck-by and eye injury hazards while performing sheathing installation operations, in that the use of eye protection was not enforced where tools such as but not limited to saws and pneumatic nail guns were in use.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall protection.   a) At 2312 Sparta Way Buford, Georgia, on or about April 19, 2023 where the framers were observed exposed to approximately 12-foot, 6-inch fall hazards while performing roof sheathing operations on the 6/12 pitch roof in that the use of personal fall protection was not enforced.
