29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips, and sparks On or about April 19, 2023, employees in the fabrication shop were performing machining operations using a lathe that did not have chuck guards and a drill press that did not have a guard. 1) Lathe#1: ACRA-Turn J.MK460 is located Mill-wright area/fab shop. 2) Bridge Port Drill Press: There is no S.N. # on the drill press located in the main fabrication area.

On or about April 19, 2023, employees in the CNC fabrication shop were performing machining operations using right-angle grinders without guards. A) Dewalt right angle grinder (no identifying marks) with no guard used in the CNC shop. B) Skil 9296 Right angle grinder (no identifying marks) with no guard used in the CNC shop.

29 CFR 1910.334(a)(2)(i): Portable cord and plug-connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). On or about April 19, 2023, employees engaged in machining operations were exposed to electrical shock hazards in various locations due to the employer not inspecting flexible cords used to power equipment. 1) Extension cord powering a thirty-two-inch Pedestal Shop fan in the CNC shop was spliced to a flat two-prong cable. 2) Extension cord powering a 4 1/2 inch angle grinder with the insulation frayed/pulled from the female end.
