Stein Highway, Seaford Middle School, Seaford, DE 19973: Employees conducted roofing activities approximately 12 feet above the grade on a timber framed pole building with a 6/12 pitch roof without fall protection. The employee went back on to the roof without fall protection fell through the metal roof onto the concrete floor below the working area.
