At a temporary worksite, located at 226 South 8th Street, Fernandina Beach, FL 32034: On or about April 19, 2023, the employer exposed an employee to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee while using a saw to cut sheets of plywood.
