On the ground, at a temporary job site located at 226 South 8th Street, Fernandina Beach, FL 32034: On or about April 19, 2023, the employer exposed an employee to head injuries in that a hard hat was not utilized by employee working underneath the other employee on the roof while handing up plywood.

At a temporary worksite, located at 226 South 8th Street, Fernandina Beach, FL 32034: On or about April 19, 2023, the employer exposed four  employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection were not utilized by the employees, while using shovels, blower and nail guns.   b. At a temporary worksite, located at 226 South 8th Street, Fernandina Beach, FL 32034: On or about April 19, 2023, the employer exposed an employee to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee while using a saw to cut sheets of plywood.

On a 7:12 pitched roof of a residential worksite,  located at 226 South 8th Street, Fernandina Beach, FL 32034:  On or about April 19, 2023, the employer exposed four employees to fall hazards ranging up to 18 feet, in that fall protection were not utilized by employees while installing sheets of plywood.
