Roofing and Siding LLC, at a jobsite located at 1178 Forest Rise Dr, Westerville, OH 43081: On or about April 20, 2023, the employer failed to initiate and maintain safety/health programs such as but not limited to personal protective equipment program, electrical safety program and a fall protection program for employees conducting roofing work.

Roofing and Siding LLC, at a jobsite located at 1178 Forest Rise Dr, Westerville, OH 43081: On or about April 20, 2023, the employer did not ensure employees wore safety glasses while utilizing portable power tools and pneumatic nailers that presented struck-by hazards to the eyes and face. Employees did not utilize eye protection. The following condition exposed employees to struck-by hazards to the eyes.

29 CFR  1926.404(b)(1)(iii)(C):Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.  a) G.D. Roofing and Siding LLC, at a jobsite located at 1178 Forest Rise Dr, Westerville, OH 43081: On or about April 20, 2023, the employer did not ensure a portable power cord used to power a circular saw was visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage and for indications of possible internal damage. Roofers used the portable power cord with missing insulation along its length and a missing ground prong, which exposed the employees to electrical contact hazards.

Roofing and Siding LLC, at a jobsite located at 1178 Forest Rise Dr, Westerville, OH 43081: On or about April 20, 2023, the employer did not ensure employees using portable power cords to provide power to a circular saw were protected from electrical contact hazards. Roofers used a portable cord without a ground prong, which exposed the employees electrical contact hazards.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.  a) G.D. Roofing and Siding LLC, at a jobsite located at 1178 Forest Rise Dr, Westerville, OH 43081: On or about April 20, 2023, the employer did not ensure employees were protected from falls while engaged in residential construction activities. Employees were performing re-roofing activities without being connected to a fall protection system. The following condition exposed employees to falls of between seven and 19 feet.
