29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide. On or about April 18, 2023, employees performed ironwork while standing on the railing of an aerial lift. The railing of an aerial lift was used as a work platform that was not 18 inches wide as required by the standard.

On or about April 17, 2023, an employee performed ironwork from an aerial lift without being anchored to the lift. The employee wore a personal fall arrest system with a lanyard but was not anchored to the aerial lift.
