29 CFR  1926.451(c)(2)(v): The employer used forklifts or similar pieces of equipment such as a rough terrain material handler to support scaffold platforms without ensuring that the forklift or similar piece of equipment was not moved horizontally while the platform was occupied:   a) Rear of Structure: An employee working on a platform supported by a rough terrain material handler was exposed to fall hazards due to the platform being moved horizontally, on or about April 20, 2023.  Abatement certification and documentation required.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) Rear of Structure: An employee was exposed to a fall hazard, in excess of 20 feet while on a platform scaffold supported by a rough terrain forklift, due to a lack of fall protection, on or about April 20, 2023. 29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system:  a) Roof: Employees installing a metal roof on a wood structure were exposed to fall hazards in excess of 25 feet from the lower surface without any type of fall protection system, on or about April 20, 2023.  Abatement certification and documentation required.
