Failure to provide personal protective equipment, including FR rated clothing, to employees exposed employees to potential thermal burn hazards inside the Baghouses.

29 CFR 1910.141(g)(2): Employees were permitted to consume food or beverage in area(s) exposed to toxic materials: MPW Industrial Services, Inc. located at 913 Bowman Street in Mansfield, Ohio 44903 - On or about April 20, 2023, the employer failed to ensure employees consuming food and beverages within the employee breakroom were protected from ingestion of heavy metals, including but not limited to chromium compounds, manganese and iron. Failure to ensure areas where employees consume food and drink are free from surface accumulation of heavy metals exposes employees to chronic health ailments including but not limited to skin irritation and other organ damage though dermal contact and ingestion.

29 CFR 1910.1026(i)(3)(i):Readily accessible washing facilities capable of removing chromium (VI) from the skin were not provided by the employer at a worksite with chromium (VI) exposures: MPW Industrial Services, Inc. located at 913 Bowman Street in Mansfield, Ohio 44903 - On or about April 20, 2023, the employer failed to ensure employees were provided with washing facilities for removing chromium (VI) from their hands prior to entering the employee breakroom to consume food and beverages. Failure to ensure washing facilities are available prior to employees consuming food and beverages exposes employees to chronic health ailments including but not limited to skin irritation and other organ damage from ingestion.
