29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 20, 2023, employees were exposed a struck by hazard while performing roofing activities on a residential home located at 9669 Sunray Drive, Olmsted Falls, Ohio 44138.  Employees were working on the roof of the home with tools and materials that could have fallen on ground employees and the employer did not ensure the ground employees were wearing protective helmets.

Employees were using air powered nail guns to install materials and the employer did not ensure they were wearing protective safety glasses.

Employees were using an extension ladder to access the roof and the employer did not ensure it was at least three feet above the landing area.
