29 CFR  1910.147(c)(7)(i) (A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) On or about April 20, 2023, at the workplace located at N60 W15288 Bobolink Avenue Menomonee Falls, WI 53051, the employer did not ensure employees received training as authorized employees before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury. The machine or equipment shall be isolated from the energy source and rendered inoperative for employees that were engaged in service and maintenance activities on equipment, such as, but not limited to resistance welding machines, press brakes and where the unexpected energizing, start up or release of stored energy could occur and cause injury.

29 CFR  1910.255(b)(4):Guarding. All press welding machine operations, where there was a possibility of the operator's fingers being under the point of operation, shall be effectively guarded by the use of a device such as an electronic eye safety circuit, two hand controls or protection similar to that prescribed for punch press operation, 1910.217. All chains, gears, operating bus linkage, and belts shall be protected by adequate guards, in accordance with 1910.219 of this part.  a.  At the workplace, in the processing area, W4/Cage welder resistance welding machine did not have the point of operation guarded, thereby exposing employees to an amputation hazard.   b.   At the workplace, in the processing area, W10 resistance welding machine did not have the point of operation guarded, thereby exposing employees to an amputation hazard.   c.   At the workplace, in the processing area, W11 resistance welding machine did not have the point of operation guarded, thereby exposing employees to an amputation hazard.   d.   At the workplace, in the processing area, W12 resistance welding machine did not have the point of operation guarded, thereby exposing employees to an amputation hazard.   e.  At the workplace, in the processing area, W15 resistance welding machine did not have the point of operation guarded, thereby exposing employees to an amputation hazard.   f.   At the workplace, in the processing area, W16 resistance welding machine did not have the point of operation guarded, thereby exposing employees to an amputation hazard.

29 CFR  1910.255(e):Maintenance. Periodic inspection shall be made by qualified maintenance personnel, and a certification record maintained. The certification record shall include the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, for the equipment inspected. The operator shall be instructed to report any equipment defects to his supervisor and the use of the equipment shall be discontinued until safety repairs have been completed. [39 FR 23502, June 27, 1974, as amended at 40 FR 18426, Apr. 28, 1975; 55 FR 13710, Apr. 11, 1990]  a.  At the workplace, periodic inspections were not performed or documented for the resistance welding equipment to identify equipment defects and hazards.

At the workplace located at N60 W15288 Bobolink Avenue Menomonee Falls, WI 53051, the employer did not conduct periodic inspections of energy control procedures annually to ensure that the requirements of this standard are being followed for employees that were engaged in activities on equipment, such as, but not limited to, automatic resistance welding machines, where the unexpected energizing, start up or release of stored energy could occur and cause injury.
