OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that employees were exposed to struck-by, caught-between, amputation, and fall hazards from defective personnel lift platforms (i.e. work platform) that were utilized on powered industrial trucks to lift personnel:  a.) Eden Inc, on or about 4/20/23 times prior thereto: a manmade wood lifting attachment was utilized on powered industrial trucks including but not limited to the TOYOTA ELECTRIC FORKLIFT Model #8FBCU25 and Serial #88109. The attachment was used for work operations including but not limited to change lights, repair airlines, and get lift personnel to get measurements. The attachment was defective in that:  1.) Mid-rail devices were not used at the platform's entranceway. 2. There was nothing on the attachment to secure the attachment to the forklift 3. The platform was not outfitted with a device to prevent personnel in the platform from becoming entangled in the power industrial trucks lifting mechanisms. 4. Mid-rail devices along the sides of the device were not constructed between the bottom of the attachment and the top rail. 5.Railing devices are not capable of withstanding 200 pounds of force  NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
