At at temporary work site located at 942 Blanch Street - Building 4, Jacksonville, Florida 32204: On or about April 21, 2023, the employer exposed employees to fall hazards, in that employees were allowed to continue to use a Werner aluminum 16-foot extension ladder that had bent rungs and a small split on the rail. The ladder was not removed from service or repaired.
