On or about 03/08/2023 at the Mystic KOA Holiday, employees were working in an 8 feet deep trench while searching for a water line, and the excavated rocks and soil were right next to the edges of the trench. No retaining devices were in use to prevent the material from falling or rolling into the excavation.
