29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  Location - 10 Clipper Way, Newburyport MA  On or about 4/21/2023, three workers without head protection were observed picking up construction debris directly below where roofers were installing new shingles, placing them at risk of head injuries from falling objects or debris.  The QGR Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1526078, citation number 1, item number 1 and was affirmed as a final order on 08/04/2021, with respect to a workplace located at 91-101 Pilgrim Circle, Methuen, MA 01844.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to hazards from flying particles.  Location - 10 Clipper Way, Newburyport MA  On or about 4/21/2023, while using a roofing coil nail gun, employees did not utilize eye or face protection in order to protect their eyes and face from flying particles.  The QGR Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1526078, citation number 1, item number 2 and was affirmed as a final order on 08/04/2021, with respect to a workplace located at 91-101 Pilgrim Circle, Methuen, MA 01844.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  Location - 10 Clipper Way, Newburyport, MA  On or about 4/21/2023, seven employees working on a steep pitched roof, at heights ranging from 9'9" to 21'6", were not protected from falls. Many employees were wearing fall protection harnesses which were not connected to any anchorage points or lifelines. There were no other fall protection measures being used.  The QGR Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1526078, citation number 1, item number 3 and was affirmed as a final order on 08/04/2021, with respect to a workplace located at 91-101 Pilgrim Circle, Methuen, MA 01844.
