1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel or the distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Employees engaged in grinding activities at the Dayton pedestal grinder located in the maintenance shop area were exposed to struck-by hazards, the distance from the abrasive wheel to the adjustable tongue guard was found to be approximately 1 and 3/4 inches.

Employees were exposed to electrical shock hazards. Metallic electrical knockout boxes were in use as electrical pendants, employees were exposed to electrical shock and electrical burns hazards in the following instances. 1) In the forklift battery exchange area. 2) In the conveyor area. 3) In the packing area.
